Questions like what medications you’re on, what is your normal blood pressure, what illnesses have you had, what insurance do you have? You can imagine how difficult it is to remember the information or where you can find it. The vial for life project is a non profit organization program to provide important medical information “that is designed to speak for you when you can’t speak for yourself.” Everything is available free of charge on www.vialoflife.com web site. · Print the form and fill it out completely. Answer only the questions you want. · Place the vial of life decal on the front of a plastic baggie (you can print this out as well and cut it out with a scissors).· You may also consider enclosing copies of your living will, Do Not Resuscitate, an EKG copy and a picture of yourself.· Place the baggie on your refrigerator door using some tape to secure the vial at eye level so it is easily seen.· Place the second decal on your front door so emergency personnel can see it.The best way would be to maintain this information on the web site. If that is not possible please see someone from St. John’s UCC Health Ministry and they will give you a copy of the form. Being safe is the key to quality care.
